Download the required forms, fill them in, and bring the signed originals with you to the Sidi Daoud point of sale when settling payment.
Documents to bring on the day of payment
To finalise the registration at our Sidi Daoud point of sale, please bring the originals of the following documents.
- Registration form duly completed and signed
- Parent / legal guardian ID card or passport
- Child's birth certificate (issued less than 3 months ago)
- 2 ID-size photos of the child
- Medical certificate of fitness to practice sport (issued less than 1 month ago)
- Notarised power of attorney if the registration is not made by one of the parents
